GUIDELINES:This folder is for visa document related uses. Feel free to organize your visa documents, which you will receive during your procedure, in this folder.

Please proceed each step in order and with care.If English is not available in certain situations, please use the Que cardthat has been provided in this folder.

MEDICAL CHECK-UP STEPS FORMAIN & SOUTH CAMPUS

1.Take a barcode picture (照片回执)from above XingAn supermarket inMain Campusand remember tokeep the receiptfor applying visa later.Sample:

2.Go to the hospital for your medical check-up. Please remember toscreenshot your booked number.

3.Take yourPASSPORT, photocopy of passport photo page and visa page (as well as your last entrance stamp), barcode photo receipt, and booking number toHealth Care Center of Guangdong International Travel(No. 207 Longkouxi Road, Tianhe District)for medical check-up.

4.After the medical check-up, return to the medical center after a week (or the day that you have been told) to take the result. And yes, you have to go and take the results.

Visa Step-to-Step forMain CampusStudents:

STEP 1.Collectdormitory certificatefrom office room located under Dorm 16. Remember to take yourSTUDENT CARD,PASSPORT,PHOTOCOPY OF PASSPORT,PICTURE PAGEandVISA PAGE(with last entrance stamp page) with you.STEP 2.Go to police station 石牌派出所(Shipai police station) to fill inthe Registration Form of Temporary Residence for Visitors.STEP 3.Take a barcode picture (照片回执)from above XingAn supermarket (兴安超市2楼照相馆)and remember tokeep the receiptfor applying visa later. (Just take the one you had if you already taken the picture for the medical check-up)

STEP 4.Take all of your documents(Registration Form of Temporary Residence for Visitors, DormitoryCertificate, Photo Receipt, Medical Checkup Result, Passport, Photocopy of Passport Photo Page and Visa Page)to Jinan University Main Campus Administration Building 130. Hand in all Documents to Ms. Hao.She will stamp yourRegistration Form of Temporary Residence for Visitorsand give youVisa and Residence Permit Application Form.Which brings us to the final step……FINAL STEP.Take yourmedical check-up report, Photo receipt, Registration Form of Temporary Residence for Visitors, Visa and Residence Permit Application Formto go through the residence procedure (visa) in:Division of Entry and Exit Administration of Guangzhou Public Security Bureau (No. 155 Jiefangnan Road) 广州市公安局出入境管理处（解放南路155号）

STEP 2.Go to Teaching Building, Room 120 and they will help you with the procedure at Xinzao police station (there is no need for you to go to Xinzao police station anymore). You will need to go back to Room 120 at the time they told you to collect yourRegistration Form of Temporary Residence for Visitors.
